Returning an Instrument for Repair

Any instruments returned to BASi® require a return authorization (RA) number. This number connects your package with an information file and expedites the handling once it arrives here. If it is not entirely clear that you need actual repairs, you should first consult with a BASi® representative.

To obtain an RA number, please contact a BASi® associate by one of the following means:

Be sure to provide the following information:

  • Model of BASi® instrument
  • Serial number
  • Your name
  • Your contact information (email, phone, FAX)
  • Your mailing address
  • Your shipping address (where should we ship the repaired instrument)
  • Description of the problem you have encountered, and the means you have used to try to correct it
  • Your purchase order number or method of payment

Decontamination Certificate

All items returned to the BASi® factory must have a completed and signed Decontamination Certificate. The certificate should be placed in an envelope inside of the box. Please print and complete this form for each instrument or part to be returned.  If a Decontamination Certificate is not included with your return a $95.00 fee will be applicable. 

Do not return items for service without a Return Authorization number for proper internal tracking. Items returned to BASi® should be shipped prepaid by the customer. BASi® then will pay shipping on the return journey. Please be sure to insure the shipment for the replacement value of the instrument, include an itemized packing list showing all items being returned, and package the item to comply with your carrier’s insurance requirements. We cannot be responsible for items which never arrived or are lost or damaged during shipment. See our packaging instructions for more hints.
